Position Description 

Patient Care Assistant 

Position Summary: 


The Patient Care Assistant works with clinic staff to organize, schedule, and perform the daily operations of the clinic to ensure comprehensive support to the clients of Northland Cares. The Patient Care Assistant engages in client-centered services such as care plan coordination, treatment adherence, medical history recording, patient care and rooming for practitioner appointments, supports linkage to medical services and referral follow-ups as needed. The Patient Care Assistant interacts with members of an interdisciplinary team of non-medical and medical personnel in a clinic setting. Minimum Qualifications: 

• Associate's Degree in Health or Social Service related field plus 2 years applicable work experience OR 5 years applicable work experience in a medical setting in lieu of degree OR  

• Any equivalent combination of experience, training, and education as approved by hiring committee and approval of Executive Director 

• Experience with Practice Fusion or other EHR program software 

• Familiarity with computer programs and data management, including MS Office products 

• Effectively present information and interact with medical providers, clients, and the general public 

• Comfort level with discussing transmission of sexually transmitted infections (STls) and safer sex behaviors for STI prevention 

• Attention to detail for effective quality management 

• Proven time management skills, organizational skills, and documentation ability 

• Excellent verbal (phone, in-person) and written communication skills 

• Excellent planning and organizational skills 

• Demonstrated diversity sensitivity 

• Demonstrated ability to maintain confidentiality 


Preferred Qualifications: 

• Familiarity with medical treatment and prevention for HIV/AIDS 

• Bilingual English/Spanish 


Supervisory Reporting Structure: 

The Clinic Assistant reports to the Clinic Manager for daily responsibilities and works under the direction of the medical practitioners for patient care.  

Scope of Work: 

• Prepares and provides patient medical documentation in compliance with all federal, state, and clinic policies and procedures. 

• Makes lab reminder calls 2 weeks prior to clinic appointment and appointment reminder calls 2 days prior to clinic days. 

• Processes patients for practitioner appointments; rooms patients and takes/records vitals; assesses patient status to report to medical practitioner. 

• Responsible for cleaning exam rooms, monitoring, ordering clinical supplies, and maintaining inventory. 

• Demonstrates the knowledge and expertise necessary to uphold standards of decontamination and sterilization of equipment and exam rooms. 

• Prepares and provides patient medical documentation in compliance with all federal, state, and clinic policies and procedures. 

• Maintains security and confidentiality of client information. 

• Facilitates clinical follow-up and referrals as directed by providers as needed. 

• Remains informed of patient status changes through communication with providers, clinical staff, outside laboratories, etc. 

• Ensures all staff members are kept current on patient status for all other services and office work flow. 

• Interacts with patients, potential patients and their families to advise of services offered by Northland Cares. 

• Contributes to team effort through task accomplishment, facilitation of communication with all clinic employees, and pro-actively advising the Executive Director of activities, needs, etc. 

• Assists in program QM activities and maintains security and confidentiality of sensitive information. 

• Documents actions in all required program database and maintains confidential patient information, ensuring accuracy, completeness and timely recording. 

• Communicates with Clinic Manager to ensure adequate office coverage and quality service. 

• Participates in regular meetings including Front Office and Staff meetings. 

• Provides rapid HIV testing and counseling within established guidelines. 

• Answers phones, schedules patients, files and performs other office functions. 

• Assists clients with scheduling/obtaining necessary lab work/results. 

• Conducts prior authorization for insurance billing and work with patients on referrals as needed. 

• Ensures patient waiting area and front office are sanitized and tidy. 

• Assists in fostering professional communications and relationships among staff. 

• Performs other duties as identified by the Medical Director and Executive Director. 

• Front and back office training as needed 


Physical Requirements: 

• Sitting, standing, and walking throughout the day for short to medium periods of time. 

• Occasional lifting of 20-50 pounds.
